Add service health indicators to LED matrix viz
Validate and Test / validate-patches (push) Failing after 13s

- Column 12 shows service health (top row = service dots)
- Periodic diagnostic flash every 10s (idle only)
- Services monitored: midi-bridge, pd-synth, viz, prometheus, audio
- MIDI note viz uses columns 0-11, rows 1-7
